Instructions: Print the form and fill in all applicable information. When complete, please mail or fax the form to us. If you are paying by credit card, you will need to fill in your credit card information at our Web Pay site: https://cfwebprod.sandia.gov/cfdocs/ccpwa/ along with sending us the registration form.

Instructions for Web Pay site: If you have your pop-up blocker turned on, please disable it. Your payment will not get processed if it is on.

In the first field "Event Name," scroll down and select "15th International Meshing Roundtable" from the drop down list. Then fill in all the other fields and click the button "Charge My Card."

If you do not receive a pop-up message with your confirmation receipt, your payment did not go through. If this happens, please make sure your pop-up blockers are disabled and try again. If they were disabled and you do not receive an automatic receipt, please call Lynn Washburn at 505-845-3520.

Registration Details
Full registration for the Meshing Roundtable is $485 ($555 after August 15, 2006).

The fee covers registration for all sessions, proceedings, CD and snacks throughout the conference. It also covers the poster luncheon on September 18th (guests excluded) and the banquet awards ceremony on September 19th (guests excluded). This registration fee does not include the short courses to be held on September 17th. The short course fee is $140 per attendee which includes course materials on all topics and lunch.

Student Registration: $130. Current student ID MUST be provided with the registration form to receive this reduced rate. Please check the appropriate place on the registration form if you are a student. Student fee covers registration for all sessions, proceedings - CD only and snacks throughout the conference. It also covers the poster luncheon on September 18th (guests excluded) and the banquet awards ceremony on September 19th (guests excluded). This registration fee does not include the short course to be held on September 17th. The short course fee is $140 per attendee which includes course materials on all topics.

Guests: Additional fees for Poster Session and Banquet Dinner
On the registration form, we have provided an opportunity to bring guests to the Poster Session which will be held at the Sheraton Birmingham Hotel during lunch and to the Dinner Banquet which will be held at The Birmingham Museum of Art on the evening of September 19th. There will be an additional charge for each guest who accompanies you to these functions. The Poster Session is an additional $30 per guest and the Banquet is $55 per guest. Children 12 and under are half price; toddlers and infants are free. Please keep in mind that these functions are covered in the registration fee for all Meshing Roundtable attendees. Indicate on the registration form if you plan to invite guests to these functions.

Hospitality
A reception will take place on Sunday, September 17, 2006, from 6:00-8:00 pm at the Sheraton Birmingham Hotel. You may pick up your registration packet at that time.

There will be a reception and tours on Monday, September 18, 2006 starting at 5:30 pm at the University of Alabama - Birmingham.

Note: Pictures will be taken at the workshop. These pictures will be included in the 15th IMR website. If you do not want your picture posted to the web, please contact Lynn Washburn or Bernadette Watts.
